    Speaker Bio

    Patrice Sminkey comes to the Commission from URAC, where she most recently served as senior director of sales. Prior to that, she was senior vice president, operations and client management, Patient Infosystems in Rochester, NY. As chief executive officer, Sminkey oversees the management of all activities related to the Commission, including leadership of the Commission's strategic vision, business development and all its programs, products and services. She holds a diploma of nursing from the Chester County School of Nursing. She is a member of the Academy of Managed Care Pharmacy, Case in Point Peer Group, Healthcare Information and Management Systems Society and Women Business Leaders of the U.S. Health Care Industry Foundation. Patricia Marine Barrett, MHSA

    Speaker Bio

    Tricia Marine Barrett joined NCQA in 2008 as Vice President for Product Development, exploring new product concepts and evolving existing products to meet the needs of a changing health care environment. Prior to joining NCQA, she was the lead consultant for General Motors on managed care. As HAP Associate Vice President and Program Director for the HAP/GM Managed Care Consulting Team, she was responsible for evaluating the quality and efficiency of GM's managed care offerings nationally and establishing supplier development activities with all of GM's HMOs. In this role, she participated on the NCQA Purchaser Advisory Council, the National Business Coalition on Health eValue8 Steering Committee and served as an author and scorer for the eValue8 RFI. She was also a member of the NCQA HEDIS Policy Panel and Chairperson for the Measurement Committee of the Michigan Quality Improvement Consortium (MQIC). Ms. Barrett attended the University of Michigan receiving her Bachelors degree in Sociology and a Masters Degree in Health Services Administration from the School of Public Health.

Kylanne Green

    Speaker Bio

    Kylanne Green is President and CEO of URAC, a national accreditation leader. A Registered Nurse and Adult Nurse Practitioner, she has more than 40 years of experience in health care delivery and insurance strategy and operations. Most recently, she served as Executive Vice President and CEO of Care Management and Health Plan Operations for Inova Health System, responsible for managed care and insurance services and health plan operations as well as care management and a range of hospital and non-hospital operations. Ms. Green served as the Chief Operating Officer of the Health Insurance Association of America, Chief Operating Officer of Aetna Health Plans of the Mid-Atlantic, and held various leadership positions at Kaiser Permanente, where she spent the first 16 years of her career. She also serves on a number of academic, community service and private industry boards, and is a past member of the Council on Graduate Medical Education.

    Dr. Dennis Schultz is the Midwest Regional Director for QuadMed, a company that manages more than 90 employer-sponsored primary care clinics. Prior to joining QuadMed, he completed residencies in Family and Occupational Medicine, served as faculty at the University of Utah and Medical College of Wisconsin, and practiced in a variety of health care settings. Dr. Schultz has extensive knowledge of accreditation and its value to the Medical Home. A surveyor for AAAHC, he has contributed expertise to numerous committees and served 12 years as a Board Member.

    Lon Berkeley has been with The Joint Commission since 1997 as Project Director for Community Health Center Accreditation. In addition, he is Co-Project Lead for the Joint Commission's Primary Care Medical Home Initiative, launched in 2011 for accredited Ambulatory Care organizations, in 2013 for accredited Hospitals/Critical Access Hospitals with primary care practices, and in 2014 for accredited Behavioral Health Care organizations providing or associated with primary care practices. Previously, Lon was Executive Director of the Illinois Primary Health Care Association which comprises over 50 health centers throughout Illinois. He has worked in public health policy, health planning, and as a community health educator. Lon received his Bachelor of Arts degree in Psychology from Yale University, and his Masters of Science in Health Policy & Management from the Harvard University School of Public Health.

    Speaker Bio

    Lisa Rajt is a Senior Health Care Analyst for the Value Partnerships Program at Blue Cross Blue Shield of Michigan. She works on the coordination and development of the award-winning Patient Centered Medical Home Program, and the expansion of care management to provider-based settings. In addition, Ms. Rajt develops initiatives to improve the quality of care for women. Prior to her work in Value Partnerships, Ms. Rajt worked in the Blues' Social Mission area, where she focused on grant-making and community development. She holds a Masters of Social Work in Community Organizing and a B.A. in Psychology and Communications, both from the University of Michigan.

    Julie Schilz received her formal clinical training as a Registered Nurse at the University of Nebraska Medical Center in Omaha, Nebraska. Her business degree is from Regis University in Denver, Colorado. Julie's experience collaborating with healthcare organizations and industry stakeholders drives her expertise in care management, quality office system redesign programs, value based reimbursement programs and critical knowledge of Patient Centered Medical Home (PCMH), Accountable Care Organizations (ACO) and Health Information Technology (HIT). Julie is the Director, Care Delivery Transformation at WellPoint. Julie serves as Executive Committee Liaison to the Patient-Centered Primary Care Collaborative (PCPCC), and in this role works in partnership with PCPCC's board, stakeholder centers and executive committee to enhance communication and help guide PCPCC's strategic direction. She is on the Editorial Board for Medical Home News. Julie is on the Advisory Board for the NCQA 2014 recognition program development.     Speaker Bio

    Dr. Christoph Diasio joined the Sandhills Pediatrics, Inc. practice in August 2001 and became a partner in the practice in 2003. He graduated from the University of North Carolina-Chapel Hill with Honors in Biology and from the University of Alabama School of Medicine (Birmingham), completing his Pediatric Internship and Residency at the University of Tennessee Health Sciences Center in Memphis at LeBonheur Children's Hospital/St Jude Children's Research Hospital/The Med. He is Board Certified in Pediatrics and on the American Academy of Pediatrics Vision 2020 Task Force and Co-Chair of the NC Pediatric Council, which seeks to smooth relationships between pediatricians and insurance companies. He is also on the executive committee of the AAP Section on Practice Management (SOAPM) and the SOAPM representative to Patient-Centered Medical Home issues, particularly the NCQA PCMH program.

Eugene P. Heslin, MD

    Speaker Bio

    Dr. Eugene Heslin is lead physician of Bridge Street Medical Group, a private practice which he began as a solo practitioner in 1992 in Saugerties, NY and now includes six primary care physicians. The practice achieved NCQA Level 3 Patient Centered Medical Home status in 2009. In 2001, Heslin became a member on the Kingston Hospital Board, and subsequently became chairman in 2006. He serves as medical director for the HealthAlliance diabetes programs and as a diplomat of the American Board of Family Medicine and the Medical Society State of New York House of Delegates. He is one of the founding members of the THINC board, as well as on the NYeC board and executive committee. Heslin earned bachelor's and master's degrees in chemistry from Fordham University, and his medical degree from the University of Texas at Houston. He completed his training in family practice at the Mid-Hudson Family Health Institute Residency program in Kingston.

    Speaker Bio

    Colette Carver is a Family Nurse Practitioner working for Sentara Healthcare as the Director of Transformation of Care. Upon completion of her masters in nursing from Duke University, she joined their Family Medicine staff. During her time at Duke and later with Carilion Clinic in Roanoke Virginia, she participated in early population health and office redesign strategies. Colette was instrumental in achieving recognition for twenty six Patient-Centered Medical Homes in western Virginia. She has been actively involved in health education as a former instructor for the University of Pennsylvania, as a Diabetes Educator, and as a preceptor for nurse practitioner students. At Sentara, Colette is working on a number of innovative services aimed to deliver excellent care beyond the boundaries of the traditional office visit through an integrated quality network of regional providers.

    Speaker Bio

    Melvina Tyson joined Sentara Healthcare System in 1991. She has worked in the primary care offices as well as the urgent care. She has also worked as a critical care nurse at our Level I Trauma Center. Most recently she has taken on the role as Project Manager for Transformation of Care Primary Care Redesign. This role involves the successful implementation of best practice processes in approximately 60 practices over the next several years.

Marie Maes-Voreis, RN, PHN, MA
Director, Health Care Homes, Department of Health, State of Minnesota, St. Paul, MN

    Speaker Bio

    Marie Maes-Voreis is Director of Health Care Homes for the State of Minnesota. She has led the implementation of Health Care Homes legislation for the Minnesota Department of Health and the Minnesota Department of Human Services as a joint government private sector project since 2008. She is also the State's Co-Lead for the community integration and practice transformation work of the Minnesota Accountable Health Model-SIM Minnesota testing grant. Previously she worked as a Director of Primary Communication systems at Park Nicollet Health Services in St. Louis Park, MN and Director of the Family Medicine clinics at Hennepin County Medical Center (HCMC) in Minneapolis. She managed successful implementation of a medical home using a multi-disciplinary team care model in family medicine and has extensive experience with implementation of the EPIC electronic health record. Marie has a Master's in Health and Human Services Administration from St. Mary's University and a BSN from the College of St. Benedict.

    Speaker Bio

    Dr. Amy Mullins is Medical Director, Quality Improvement for the American Academy of Family Physicians (AAFP). She has 14 years of experience as a family physician, including expertise in practice transformation to the patient-centered medical home model. Most recently she was both a practicing family physician and as medical director at Trinity Clinic in Whitehouse, Texas. While there, Dr. Mullins helped lead her practice to a level III recognition from NCQA. She also served as a physician champion for Trinity Mother Frances Health System where she facilitated system-wide medical home implementation. Her current responsibilities with the AAFP include liaison work with other national organizations in the quality arena, as well as national-level policy and advocacy work emphasizing the PCMH. A native of Texas, she earned her Bachelor of Science degree at Abilene Christian University and her medical degree from UTMB in Galveston.

David B. Nash, MD, MBA, FACP
Dean, Jefferson School of Population Health, Thomas Jefferson University, Philadelphia, PA (Co Chair)

    Speaker Bio

    David Nash is Founding Dean and the Dr. Raymond C. and Doris N. Grandon Professor of Health Policy at the Jefferson School of Population Health (JSPH) of Thomas Jefferson University. A board certified internist internationally recognized for his work in outcomes management, medical staff development and quality-of-care improvement, in 2006, he received the Elliot Stone Award for leadership in public accountability for health data from NAHDO. In 2009, Dr. Nash received the Wharton Healthcare Alumni Achievement Award. He has chaired the Technical Advisory Group of the Pennsylvania Health Care Cost Containment Council for a decade where he helped to pioneer public reporting of outcomes. In December 2009 he was named to the Board of Directors for Humana Inc. In March 2011, he joined the Board of Directors of Endo Pharmaceuticals.
Marci Nielsen, PhD, MPH
Chief Executive Officer, Patient-Centered Primary Care Collaborative, Washington, DC (Co Chair)

    Speaker Bio

    Dr. Marci Nielsen is Chief Executive Officer of the Patient-Centered Primary Care Collaborative (PCPCC). She previously served as Vice Chancellor for Public Affairs and Associate Professor within the Department of Health Policy and Management at the University of Kansas School of Medicine, where she was twice awarded Professor of the Year by her students. She also served on the Board of Directors of the Health Care Foundation of Greater Kansas City, TransforMED, and the MidAmerica Coalition on Health Care. She also has been a committee member on the Institute of Medicine's Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action. She serves as the first public (non-physician) member on the American Board of Family Medicine. Prior to rejoining the KU faculty, she was appointed by then-Governor Kathleen Sebelius to run the Kansas Health Policy Authority (KHPA). At the national level, she has worked as a legislative assistant to U.S Senator Bob Kerrey (D-Nebraska) and as the health lobbyist and assistant director of legislation for the AFL-CIO. She earned her MPH degree from George Washington University and her PhD in Health Policy and Management from the Johns Hopkins School of Public Health.

New York State and the Patient-Centered Medical Home: A Glimpse into the Future of Health Care

Nirav R. Shah, MD, MPH
Commissioner of Health, State of New York, Albany, NY

    Speaker Bio

    Dr. Nirav R. Shah is the 15th Commissioner of the New York State Department of Health. He graduated with honors from Harvard College, and has an MD and MPH in medicine and chronic disease epidemiology from Yale University. He is board certified in Internal Medicine and is an elected member of the Institute of Medicine (IOM). Dr. Shah heads one of the world's leading public health agencies with an annual budget of more than $58 billion. During Dr. Shah's tenure, the Department led the transformation of the state's Medicaid program, which resulted in more than $4 billion in savings in just the first year while improving population health and quality of care. The Department also spearheaded the creation of a health benefit exchange that will give 1.1 million New Yorkers health insurance coverage and drafted an evidence-based Prevention Agenda for improving the health of all New Yorkers.

    Speaker Bio

    Bill Copeland is the Vice Chairman, U.S. Life Sciences and Health Care Leader for Deloitte LLP and the sector leader of the practice's health plans group. A 27-year veteran with Deloitte, Bill has advised some of the organization's largest health care industry clients on their most strategic and transformative events, including mergers, affiliations, new business initiatives and financial and operational turnaround plans. During his career, Bill has served more than 50 health plans in 40 different markets. Bill's previous experience includes starting a national physician practice management company through venture capital funding and strategic partner investments of over $100 million and serving as chief executive officer of a major health care provider. Bill has a master's degree in business administration from the University of Notre Dame and a bachelor's degree in psychology and economics from the University of Dayton.

    Dr. Terrence O'Rourke is the Executive Vice President Clinical Transformation for CHE Trinity Health and responsible for advancing clinical effectiveness, quality and patient safety. Dr. O'Rourke was the Chief Medical Officer at Centura Health and a practicing general surgeon in Colorado. After graduating from Georgetown University and the University of Michigan, he was Chief of Surgery at the US Army Hospital in Colorado. Dr. O'Rourke is a member of several medical and surgical associations. He has served on numerous boards of healthcare organizations. He was recognized with the Sword of Hope Award from the American Cancer Society in 1992 and the "Best Doctor in America" award in 2000.

    DR. TOM GRAF is the CMO for Population Health and Longitudinal Care Service Lines for Geisinger Health System. Dr. Graf is responsible for the Value Re-Engineering of the Care Continuum and other population health initiatives for Geisinger including the ACO portfolio and with CMS, the Physician Group Practice Transitions Demonstration and Bundled Payments for Care Improvement. He leads the Community Practice, Internal Medicine, Pediatrics, Psychiatry and Care Continuum Service Lines in coordinating and accelerating population health related activities across 22 counties in central and northeast Pennsylvania. He is recognized nationally as a leader in medical home and post-acute care redesign.

    Dr. Martin Sepulveda is an IBM Fellow, the highest level of technical achievement in the IBM Corporation and its Vice President of Integrated Health Services, responsible for health care policy, strategy and design, as well as the management system and services supporting the health and well-being of IBM's workforce and work environments. He is also a fellow of the American College of Physicians, the American College of Occupational and Environmental Medicine, and the American College of Preventive Medicine. He serves on the National Academy of Sciences' Institute of Medicine's Population Health and Public Health Practice Board, the Board of Directors of the Employee Benefits Research Institute, the Board of Advisors to the School of Public Health at the University of Iowa, the Board of the National Business Group on Health, and chairs the Global Health Benefits Institute. He received his MD and MPH degrees from Harvard University, completed internal medicine residency at the University of California San Francisco Hospitals & Clinics, internal medicine fellowship at the University of Iowa Hospitals & Clinics, occupational medicine residency at NIOSH, CDC, and Epidemic Intelligence Service at the Centers for Disease Control & Prevention.
